Pattern of Retinopathy: Although the locus of toxic damage is parafoveal in many eyes, Asian patients often show an extramacular pattern of damage. Dose: We recommend a maximum daily HCQ use of 5.0 mg/kg real weight, which correlates better with risk than ideal weight.

He pointed out that these findings have been validated in several studies using other patient populations. He concluded that the risk is very real of developing retinal toxicity with prolonged use of hydroxychloroquine. The dose, duration, presence of renal disease and use of other drugs such as tamoxifen all contribute to the increased risk. The Hopkins team seems to have found a workable middle ground – weight-based dosing up to a maximum of 400 mg/day, which is considered safe in a lean person of about 136 pounds. Three months after being prescribed hydroxychloroquine, however, just 56% of the patients were at a therapeutic level of 500 ng/mL or more. A risk factor for Plaquenil hydroxychloroquine retinotoxicity is a daily dose that exceeds 5.0 mg of drug per kg of body weight. The tool on the right simply calculates this threshold based on a ppatient’s real body weight. It’s important to understand that the daily dose is only one risk factor for plaquenil retinotoxicity.

Risk of Toxicity: The risk of toxicity is dependent on daily dose and duration of use. There are no similar demographic data for CQ, but dose comparisons in older literature suggest using 2.3 mg/kg real weight.

The risk increases with a higher dose and longer therapy. Vision risks. Hydroxychloroquine is dosed according to body weight. The American Academy of Ophthalmology AAO currently defines an overdose of hydroxychloroquine as more than 5.0 milligrams mg per kilogram kg of body weight 1 kg equals 2.2 pounds or more than 400 mg a day. Background/Purpose Hydroxychloroquine HCQ is a widely-used medication in many rheumatologic conditions. The most worrisome toxicity is irreversible damage to retinal pigment epithelium which can lead to blindness. The most recent 2016 guidelines from the American Academy of Ophthalmology AAO recommend the dose of HCQ ≤5 mg/kg real body weight to minimize toxicity1. According to these Chloroquine is often part of a multi-drug regimen with other anti-parasitic medications. A cumulative dose of over 460 g or a daily dose of 2.3 mg/kg body weight/day is high risk. The latest studies show that using the patient's real weight instead of ideal weight correlates better with safe dosing. Chloroquine is rarely an option for rheumatic.

Hydroxychloroquine causing retinal damage My husband. - NRAS He said they do tests on the NHS for people on Hydroxy all the time. He had come across retinal damage after a few months caused by Hydroxy but not three weeks but would not rule it out. I told the consultant in May 2016 about the retinal spot and he said it only happens after about ten years and I was on a low dose.
